At Hue-MAN, health includes not only physical and mental health but also trust, equity, and the social determinants of health that shape people’s lives. We ground our work in four interconnected priorities:

This work happens through deep, long-standing partnerships with major institutions and dozens of community groups, organizations, and individuals who make Hue-MAN’s mission possible.

Community Outreach Projects

City of Minneapolis Mobile Medical Unit

Hue-Man Partnership is proud to support the City of Minneapolis Mobile Medical Unit (MMU), a vital initiative bringing healthcare directly to where people live, work, and gather. Staffed by licensed healthcare professionals, the MMU provides free basic health screenings, wound care, medication consultations, and mental health counseling and referrals. The unit also helps connect residents with longer-term care and community resources.

Coming soon, expanded services will include dental care, vaccinations, and medications for opioid use disorder.

This work is made possible by collaborations with the City of Minneapolis and the Northside Urban Coalition.

Through these partnerships, Hue-Man continues to advance health equity by reducing barriers to care and building trust through community-centered outreach.

The Daddy Book: A Guide for New Fathers

This interactive workbook was created to support new fathers during their baby’s first year of life. Its purpose is to help fathers build strong, positive relationships with their babies, understand infant development, and navigate the emotional journey of becoming a parent. It also aims to help feathers define their role in their family, including coparenting relationships.

This book was originally created in the 1990s and has been updated over the years, most recently in 2025. Hue-MAN plans to continue the series, developing additional resources to support fathers and families.

This new edition was made possible with support from the Birth Equity Community Council, Healthy Black Fathers, University of Minnesota Extension, and the City of Minneapolis, among others.

Partners in Prevention Clinic

The Partners in Prevention (PIP) Clinic is a community-based public health nursing program co-founded by Hue-MAN Partnership and Metropolitan State University . The clinic brings free, prevention-focused services directly into trusted community spaces, with realtime connection to followup resources.

Between fall 2022 and spring 2025, the PIP Clinic:

  • Delivered 1,361 one-on-one service encounters

  • Completed nearly 2,000 blood pressure checks,

  • Linked community members to health and social services in real or near time

  • Achieved 100% satisfaction, with participants reporting they felt respected and heard

PIP also serves as a real-world practicum site for nursing students, helping build a caring, culturally responsive public health workforce.

By bringing essential care directly into the community, this initiative reduces barriers that prevent people from achieving their optimal health. It is part of Hue-MAN’s larger mission to advance health equity in the community.

More than 30 learners graduated from our youth substance abuse training in June 2023. The program is designed to help young people provided peer-to-peer counseling.

The four-day training included education about the opioid epidemic and how to administer Narcan (naloxone), a medication used to reverse the effects of an opioid overdose.

Hue-MAN plans to bring these youth to community events as trained ambassadors. The youth say they’re interested in giving back to their communities using the knowledge and skills they’ve learned.

This program is funded by a grant from the City of Minneapolis.

Hue-MAN Community Substance Abuse Training

The MN 2035 Plan

The MN 2035 Plan is Minnesota’s statewide strategy to reduce cardiovascular disease, stroke, and diabetes — conditions that disproportionately impact communities of color. Developed with over 90 organizations and community voices, the plan aims to eliminate health inequities, remove barriers to well-being, and improve access to culturally relevant prevention and care.

The plan emphasizes community-led solutions and partnerships beyond traditional healthcare settings. Hue-Man Partnership plays a vital role by engaging and uplifting communities, building trust, and supporting culturally grounded programs that make healthy living more accessible.

Through collaborative efforts like these, the MN 2035 Plan empowers Minnesotans to create a healthier, more equitable future together.

Clarence Jones, Hue-MAN executive director and community health strategist, was a co-author of this Plan, published in MInnesota Physician.

Health Care in Minnesota: Summary Report on Quality, Disparities, and Cost

The 2025 MN Community Measurement (MNCM) report analyzes Minnesota’s health care quality, disparities, and costs for the 2023 measurement year. While Minnesota maintains strong overall health indicators, the report reveals persistent gaps in care and significant disparities affecting communities of color and immigrants.

Hue-MAN Partnership contributed community insights and advocated for culturally relevant approaches to addressing health disparities highlighted in the report.

By partnering with academic powerhouses, like Mayo Clinic and the University of Minnesota, our work combines academic rigor and deep community engagement. We focus on listening first and acting together, setting the standard for health equity work driven by community priorities.
